The tradition. In the tradition, a sign to be careful: worry, a delay, an argument at home or an unexpected expense. Many lists read an upper eyelid twitch as good news, whichever eye it is. Here the two rules disagree. Most families go by which eye twitches; the eyelid variant is the less common one.

Health. Medically, an eyelid twitch is usually myokymia: small, harmless contractions of the eyelid muscle, brought on by stress, short sleep, too much tea or coffee, or long hours at a screen. It mostly settles within days. See a doctor, ideally an eye specialist, if the twitching lasts beyond about two weeks, spreads to the cheek or mouth, the eyelid droops or will not open, or the eye is red or painful or your vision changes. Get emergency help at once if one side of the face suddenly droops, an arm or leg goes weak, or speech slurs.

In Shakun Shastra, the lore of omens, a throbbing of the body (anga sphurana, अंग फड़कना) on the right side is auspicious for a man and on the left side for a woman, and the other side calls for care. It then adds the eyelid variant many lists carry, and says plainly when the two rules disagree.
