Does the name have to start with one of these sounds?

No. This is a tradition, not a rule, and no text treats a name chosen for another reason as harmful. Families regularly name a child after a grandparent or a deity instead. The only consequence is that the name will not encode the birth star, so nobody will be able to read the chart back out of it later.
